Use your cell phone to scan the check-in QR code when you arrive at the office!
You will be asked to enter your name and date of birth. Your identifying information is authenticated with your cell phone number. A registration interview then appears on your cell phone screen, prompting you to complete any necessary intake tasks, including:
- Entering updated or missing demographics
- Signing consents
- Capturing pictures of photo ID and insurance card
- Make payments.
Once registration is completed, the front desk staff is notified that the patient has completed the check-in and is ready.
